Photo Consent: I/we give permission to the Lethbridge Diving Club to publish/proint photos of my/our child (without names) for use in club advertising and on the website.

Freedom of Information Consent: I/we give permission to the Lethbridge Diving Club to use any information received at registration for Lethbridge Diving Club purposes only.

Code of Conduct: The Lethbridge Diving Club has established rules for participation and conduct on and about the diving area in order to protect the safety of the divers, coaches, and observers. This checkbox acknowledges that all participants and parents agree to follow these rules.

Waiver: It is clearly understood that the Lethbridge Diving Club and/or its Board of Directors will not be held responsible for any accident or loss, however caused, and that the applicant agrees to release the said club, its Board and the proprietors from all liability, claims and damages which may arise as a result of, or by reason of, such accident or loss.

Upon submitting this form, you will be taken to our Bingo Registration area to sign up for the bingo's that you will be required to work in order to support the club.

Each family is required to work 3 bingo's per diver for children who dive one night per week and 5 bingo's per diver for children who dive two nights per week.
